When requesting forms to be completed, please ensure that all patient sections are fully completed, signed, and dated—such as the consent for the release of medical information—and that any relevant supporting documents are attached before dropping off the forms.
Forms dropped off , faxed or submitted electronically with missing signatures, incomplete patient sections, or lacking essential supporting documents will lose their place in the queue and be moved to the bottom of the list.

Forms need to be brought in so that we can discuss them. If you have forms for the physician to fill out or need a note, providing them is at the discretion of the physician as different forms have different complexity. Chart review may be needed and typically this requires dedicated time slots. You should always inform the booking staff if you have any form need to fill up before your appointment.
They will NOT be completed during the appointment and as per CPSO guidelines they will be completed within 60 days.
Please plan well in advance if you need these kinds of requests, sometimes certain forms are associated with a deadline date, and you may need to request an extension of it from the requesting party.

The Ontario Health Insurance Plan (OHIP) provides coverage for most BUT NOT ALL physician services. Examples of Uninsured services such as sick notes, online booking, medical advice via email, missed visit fees, Prescription refills without a visit(assessment), TB tests, liquid nitrogen treatment, third-party physicals, chart transfers, form completion, notes and letters, and some vaccines will be invoiced at Ontario Medical Association rates. These fees are subject to change with short notice.

We kindly ask you to make your payment upon request as the clinic is too busy and burdened with admin to chase bills.
