What other General Practice Policies I need to know?

We take the confidentiality of our patients’ health information extremely seriously. Unless required by law, we will not release any details regarding your/your child’s health without your prior consent. 

This office is fully compliant with The Ontario Government’s Personal Health Information Protection Act, and our computerized records system is protected against loss of data and unauthorized access. 

If you are seeking in-person care at the clinic, our staff will confirm you read the screening questions, If you are sick you need to have a negative COVID test before you visit us.

We would be grateful if you would take the time to discuss with us any suggestions you have about our service to you. We run this practice for our patients and your feedback is very important to us. 

We are working very hard to create a safe and efficient medical clinic. However, we understand that these policies may not be suitable for some people. If you need to find a new family physician, please visit Health Care Connect, or call 1-800-445-1822 for assistance. 

Rude, aggressive and/or intimidating behavior towards staff will not be tolerated and will be immediate grounds for dismissal from the practice. 

Unfortunately, no switching over option is available to other FHO providers in the group.
