If you booked online, please use the original confirmation email to cancel the appointment 24/7. Keep this email as within it is the option to cancel the appt. 24-hour notice is now required. Otherwise, just call the office and one of our staff members will be happy to help you.
The appointment you have means that we have reserved time for you. We kindly request 24 hours' notice to cancel or to reschedule an appointment. If you miss an appointment without 24 hours notice, you may be billed for the missed visit at Ontario Medical Association recommended rates. Click here for more details. You will also be receiving our new uninsured services policy via mail shortly.
Please consider that sometimes you may not need an appointment, your doctor will review those requests case by case and action them as appropriate.
The average response time for these requests is 3-5 business days.
Examples are:
• Prescription refill without assessment. (*Fee may apply)
• Prescriptions for insurance benefits purposes (Physiotherapy/ Massage/Custom Orthotics, Braces, etc.) (*Fee may apply)
• Parking permit form.
• Patient received Cancer screening letters (FIT test, Mammography,)
• Other Miscellaneous Types of short Forms (Special Diet or similar)
• Report an Immunization / Vaccination
• Referral for PAP test by a female practitioner.
• Pay your annual block fee. (*Fee may apply)
